​How to Handle Constipation from GLP-1 Medications 

Constipation is a common side effect experienced by individuals taking GLP-1 medications such as Ozempic, Wegovy, Mounjaro, and Zepbound. These medications work by slowing gastric emptying and reducing appetite, which can inadvertently lead to digestive issues, including constipation. At Trevi Medical, we prioritize a comprehensive approach to managing this side effect, ensuring our patients' comfort and the effectiveness of their weight loss journey.​ 

Understanding the Cause 

GLP-1 receptor agonists, while effective for weight loss and blood sugar control, can slow down the movement of food through the digestive tract. This delayed gastric emptying can result in fewer bowel movements and harder stools. Additionally, reduced food intake may lead to lower fiber consumption, further exacerbating constipation. ​  

Trevi Medical's Approach to Managing Constipation 

Our strategy encompasses dietary adjustments, hydration, supplementation, and, when necessary, medication modifications:​ 

1. Increase Fiber Intake 

Incorporating high-fiber foods such as leafy greens, legumes, and whole grains can add bulk to the stool and promote regular bowel movements. For those struggling to meet fiber requirements through diet alone, fiber supplements like psyllium husk may be beneficial.   

2. Stay Hydrated 

Adequate water intake is crucial. We recommend consuming at least 8–10 glasses of water daily to help soften stools and facilitate bowel movements. ​  

3. Magnesium Citrate Supplementation 

Magnesium citrate acts as an osmotic laxative, drawing water into the intestines and easing stool passage. It's a gentle option we often suggest to alleviate constipation.​ 

4. Monitor and Adjust Medication Dosage 

If constipation persists, we assess the GLP-1 medication dosage. Sometimes, a temporary dose reduction can alleviate symptoms. Additionally, we may introduce other peptides that support fat burning without intensifying gastrointestinal side effects.​  

5. Regular Physical Activity 

Engaging in regular exercise stimulates intestinal activity. Even moderate activities like walking can significantly improve bowel regularity. ​  

6. Consideration of Other Medications 

In cases where lifestyle modifications are insufficient, we may explore medications such as lactulose, linaclotide (Linzess), or lubiprostone (Amitiza) to manage constipation effectively.
